DIFFERENT CIVIL STATUS CERTIFICATES
You may request:
- a birth certificate
- a marriage certificate
- a death certificate
The certificate can be requested in 3 manners: certificate without direct line of descent, with direct line of descent or the long form.
To obtain a long form certificate or a birth or marriage certificate with direct line of descent, the person requesting the certificate must meet 3 requirements: Be the person the act concerns, his/her spouse, partner of a civil union, his/her ancestors or descendants, his/her legal representative,
- Be of legal age or an emancipated minor,
- Be able to indicate, when asked, the usual names and first names of the person’s parents.
To obtain a long form copy of a death certificate, the person requesting the certificate must indicate the date and place of death, and the name and first names of the deceased person. A death certificate can be issued to any person requesting it. You can carry out your request at the relevant City Hall:
by mail or at the office.

When your certificate is issued, please bring your identity card with you. You must also bring the family record book if you are not the person concerned in order to justify your filiation.
Requests for a civil status certificate cannot be made by telephone
